Can someone tell me why some images have a comment box available at the bottom of the page and for others you have to click on the 'Add a comment' link? I've looked through the options and can't see anything that is obvious to enable this.

I depends on what kind of style sheet you use. In the gallery edit mode you can choose different style sheets or add your own like I did. Some style sheets have the comment box, for others you have to click for it. Personally I like having it as it makes it easier for the viewer and it's one less click.
