HELP!!! Case study for MA degree- short survey!

Hiyas PAD'r crew.

I know I'm usually more of a lurker in the forum.. However, I am working on my Master's degree right now. (graduate in July I hope) I have two case studies due and I need to gather information/data for them. My case study is on how PBASE/PAD is a Community of Practice.

It would be Fantastic if as many people as possible could take my multiple choice survey. (yes, ASAP-I really should have done this sooner) (9 questions only-I kept it short!) And yes, it's annoymous!
